Do you use prescription medication more than 12 times per year? Plan to use HRT and want to save a little money?
If so, an NHS Prescription Prepayment Certificate (PPC) could save you money.
What is the NHS Prescription Prepayment Certificate?
A PPC is like a season ticket for prescriptions. Instead of paying each time, you pay a fixed fee upfront and get unlimited NHS prescriptions in England for the duration of the certificate.
From April 2025, the prices are:
- £32.05 for a 3-month PPC
- £114.50 for a 12-month PPC
You can pay the annual fee in one go, or in 10 monthly instalments via direct debit.
When you collect prescriptions, simply show your certificate (digital or paper) and you won’t pay anything further.
The HRT Prescription Prepayment Certificate (HRT PPC)
If you’re prescribed hormone replacement therapy (HRT), you can apply for a specific HRT PPC.
This costs just £19.80 per year and covers unlimited eligible HRT medicines.
It’s ideal for anyone who only needs HRT medicines, though if you take other prescriptions as well, a standard PPC might offer better value.
When does a PPC save you money?
At £9.90 per item (the standard NHS prescription charge in England for 2025/26):
- A 3-month PPC pays off if you need 4 or more prescriptions in that time.
- A 12-month PPC pays off if you need 12 or more prescriptions in a year.
Anything above this, and you’re effectively getting prescriptions free. For people on multiple medications, the savings can be hundreds of pounds a year.
How to buy a PPC
You can buy a PPC:
- Online at the NHS Business Services Authority website
- By phone on 0300 330 1341
- At some pharmacies
Your certificate can be issued digitally (via email) or by post.
Refunds and flexibility
- If you pay for a prescription while waiting for your PPC, ask for an FP57 receipt from the pharmacy, you may be able to claim a refund.
- If your circumstances change (for example, you become eligible for free prescriptions), you can apply for a refund for the remaining months.
Alternatives to a PPC
Not everyone needs to pay for prescriptions.
You may already qualify for free prescriptions if you are:
- Under 16 (or under 19 in full-time education)
- Over 60
- Pregnant or have had a baby in the last 12 months
- On certain benefits (e.g. Universal Credit, Income Support)
- Holding a valid medical exemption certificate
If your income is low, you may also qualify for the NHS Low Income Scheme (HC2 or HC3 certificates), which can reduce or remove charges.
Key benefits of a PPC
- Saves money for anyone needing 12+ prescriptions a year
- Convenient, no more paying each time at the pharmacy
- Flexible, choose 3-month or 12-month duration
- Refundable if you later qualify for free prescriptions
- Supports budgeting with the option to pay monthly
Save money while still getting your medication
If you’re on regular medication, an NHS Prescription Prepayment Certificate is one of the easiest ways to save money and reduce hassle at the pharmacy.
With frozen prices for 2025/26, it’s a good time to lock in savings and take the stress out of managing healthcare costs.
NHS PPC FAQs
How many prescriptions do I need for a PPC to be worth it?
If you have more than 12 items a year, a 12-month PPC will save you money. For short-term needs, 4+ items in 3 months makes the 3-month PPC worthwhile.
Can I spread the cost of a PPC?
Yes. A 12-month PPC can be paid in 10 direct debit instalments, making it easier to budget.
What is the difference between a PPC and an HRT PPC?
A PPC covers all prescriptions, while the HRT PPC (£19.80/year) only covers eligible HRT medicines. If you take other prescriptions alongside HRT, a standard PPC may be better value.
How do I claim a refund if I paid for prescriptions before my PPC started?
Ask your pharmacist for an FP57 receipt when paying. You can then apply for a refund once your PPC is active.
Does a PPC cover dental or optical NHS charges?
No. A PPC only covers NHS prescriptions. Dental treatment, eye tests, and wigs/appliances have separate charges and exemptions.
